	How does a club receive a certificate of insurance for a facility?
	Club director needs to register on-line and pay as a member. Register club profile with GP Office. We will send a certificate of insurance for the club. Fill out the Certificate of Insurance Request form for each facility who is requesting to be a certificate holder. (see form above)
